The professional team at the Utah Fertility Center is a highly experienced and talented group of kind, caring individuals. The staff of UFC is committed to assisting patients to achieve their ultimate goal – the birth of a child.
 

Russell A. Foulk, M.D.
Dr. Foulk has a passion for helping couples to overcome infertility.  Despite being an internationally recognized expert on infertility, he takes great pride in developing personal relationships with each of his patients. He firmly believes that one of the greatest joys in life is helping a couple to find hope and eventually overcome the devastating disease of infertility.

Shawn E. Gurtcheff, M.D.
Dr. Shawn Elizabeth Gurtcheff comes to the Utah Fertility Center from a thriving infertility practice at Seattle Reproductive Medicine in Seattle, Washington. She is thrilled to return to Utah, where she completed her subspecialty fellowship training in Reproductive Endocrinology & Infertility. While at the University of Utah, she also earned a Master of Science Degree in Clinical Investigation.

Scott J. Whitten, M.D.
Dr. Whitten is a board certified subspecialist in reproductive endocrinology and infertility. He focuses on helping infertile couples to achieve pregnancy, utilizing both low and high-tech therapies, as well as treat female reproductive disorders such as endometriosis and fibroids.

Cristin C. Slater, M.D.
Dr. Cristin Slater is board certified in reproductive endocrinology and fertility.  She is dedicated to helping her patients reach their goal of having a baby.  After completing her reproductive endocrinology and infertility fellowship in 1999, she initially stayed on staff at the University of Southern California where she practiced both in an academic setting and in a private practice setting.  She has been solely in private practice since 2002, and began treating patients in Utah in 2006.

CLINICAL STAFF

Anne-Marie Martin Lloyd, BSN, RN, MSN, WHNP
Anne-Marie is a women’s health nurse practitioner and a coordinator for our patients undergoing IVF cycles, Non-ART cycles, ovum donor cycles, and gestational carrier cycles. Anne-Marie graduated from Brigham Young University with a bachelor of science in nursing. After working as a registered nurse in a newborn intensive care unit in Salt Lake City, she returned to academia to pursue a master of science in nursing. Anne-Marie specialized in women’s health and graduated from the University of Utah in 2004. She joined the Utah Fertility Center team in 2005 with prior reproductive medicine experience. As a nurse practitioner, Anne-Marie offers preventative healthcare for women and coordinates and provides care to all patients utilizing assisted reproductive technology.
 
Mandy Johnson, WHNP, MSN
Mandy Johnson, MSN, WHNP, is a Women's Health Nurse Practitioner and joined Utah Fertility Center in 2010.  Prior to joining Utah FertilityCenter, Mandy practiced in Obstetrics and Gynecology in Riverton, Utah.  Mandy graduated as a Registered Nurse from Weber State University in 1999, and received her Bachelor of Science in Nursing in 2001.  After working in orthopedics and women's health she returned to academia to pursue a master of Science in Nursing.  Mandy specialized in Women's Health and graduated from the University of Utah in 2004 as a Women's Health Nurse Practitioner.  As a nurse practitioner, Mandy offers preventative healthcare for women and coordinated and provides care to patients utilizing assisted reproductive technology.
                     
Brooke Welsh, MSN, WHNP
Brooke is a Women's Health Nurse Practitioner and joined the Utah Fertility Center in 2011. Brooke graduated from Brigham Young University Idaho with a bachelor of science in nursing. After working as a registered nurse on a labor and delivery unit in Provo, she returned to academia to pursue a master of science in nursing. Brooke specialized in Women's Health and graduated from the University of Utah in 2011 as a Women's Health Nurse Practitioner. As a WHNP, Brooke offers preventative healthcare for women and coordinates and provides care to all patients utilizing assisted reproductive technology.
               
Marray Chase, Medical Assistant
Marray joined UFC in July of 2010 as our full-time CMA. Marray graduated Magna Cum Laude in the Certified Medical Assisting Program from Broadview University and is a member of the AAMA. She assists the physicians in all of their procedures and coordination, and is also the coordinator of all non-ART patients. Marray also assists with egg retrievals, embryo transfers, phlebotomy, patient education, shot class instruction, and optimizing patient care. She is very dedicated to her patients and their well-being, and has the ultimate goal of being able to help patients experience the same joy that Marray has with her own family.
               
Tonya Canaday, IVF Coordinator
Tonya came to UFC in 2011 from the Idaho Center for Reproductive Medicine in Boise. She serves as UFC's IVF coordinator. Tonya has a very diverse background of career experiences, including performing phlebotomy and andrology laboratory duties. Tonya loves being able to help our patients along their journey to parenthood and is a constant ray of sunshine in our office! 

 
LABORATORY STAFF
Lucy J. Williams, ELD-Embryology Lab Director
Lucy is an extraordinary embryologist, both experienced and certified (by ABB) in all aspects of the Assisted Reproductive Technologies laboratories of Embryology and Andrology. She graduated from Newman University in Kansas and continued to study and perform embryology at the University of Minnesota, the University of California at San Diego, the University of Texas Southwestern Medical School, and the Jones Institute, Eastern Virginia School of Medicine. Lucy has 20+ years of experience in the field and is proficient in all lab procedures, including the micromanipulation techniques of ICSI, assisted hatching, and PGD. She is an active participant in post-graduate courses to keep the lab current with the latest developments, such as vitrification of embryos. Lucy is an active member of the American Society for Reproductive Medicine, American Board of Bioanalysts (ABB), Society for Reproductive Technologies, and the Society for Reproductive Biological Technologists. She is a published author of several manuscripts.
                                 
Chelsea Foulk, Certified Phlebotomy Technician (CPT)
Chelsea is attending Utah Valley University and will be graduating with a Bachelor of Science degree in English Literature in December, 2011. She was certified with the Utah School of Phlebotomy, and has been with the Utah Fertility Center since May of 2010. Chelsea is responsible for patient blood draws and running tests for hormone levels, as well as various other patient services. She also has an eclectic array of hobbies to keep her busy in her spare time. Chelsea is an avid rock climber, a certified mountaineer, a professional Polynesian dancer, pottery thrower, and painter. Though she is obtaining a degree in English, her plan is to remain in the medical field.

FRONT OFFICE
 
Diane Fletcher, Clinic Liaison

Diane joined UFC in January of 2011. She grew up in Payson, attended Utah State University, and graduated from Weber State University with a bachelor's degree. Diane is excited to be a part of the world class team here at UFC! She is proud to represent UFC's excellent patient care and advances reproductive treatment options to our community, local physicians and providers, and to each individual struggling with infertility. Diane also assists our IVF patients who are traveling to UFC for their treatment. She has a personal understanding of our IVF patients, as she herself has experienced the disappointments along with the elations and tremendous gratitude. Diane loves spending time with her family of 5 boys - including her husband. She enjoys camping and boating, playing tennis, reading, listening to the imaginative stories and thoughts of her children, and obsessively pulling weeds (also known as gardening).

 
         
Bonnie Lees, Patient Services
Bonnie is attending Broadview University and will graduate with honors in December 2011 in the medical coding and billing field. She has always enjoyed helping others, and landed in the medical field naturally. Bonnie helps all of our patients feel at home and assists in making their experience with our office as smooth as possible. In her spare time, Bonnie enjoys outdoor activities with her husband and two children, as well as crafting and taking photos.
                    
Mindy Fierros, Patient Services
Mindy is part of UFC's front office team and is one of the first smiling faces that our patients see. She joined the Utah Fertility Center in June of 2011. Mindy plays an integral role in the office, answering all incoming calls and assisting in scheduling appointments, as well as directing calls to those who can best attend to a patient's needs. She will also gladly get our patients checked in and out in a timely fashion. Mindy is a collector of any items that pertain to a chef, as her kitchen attests. She is happily married and keeps busy with her three children. 
 
ADMINISTRATION
 
Genie Andrews, Administrator
Genie came to UFC in 2011 with over 25 years of medical management and financial counseling experience in OB/GYN, family practice, internal medicine and pediatrics. The most enjoyable part of her career has been the last 10+ years working in the reproductive field. If you have any questions regarding the financial part of your fertility treatments, Genie will be happy to discuss your options with you. Genie attended Stephens College in Columbia, MO. She is a past member of ACPC’s and IMGMA, a current member of ASRM and ARM and a sustaining member of AJL.
                
Timothy Garcia-Jay, MHA, Executive Director
Tim is the Executive Director of the Nevada Center for Reproductive Medicine, the Idaho Center for Reproductive Medicine, and the Utah Fertility Center. Tim joined the three practices in 2010 and brings over 13 years of heathcare administrative experience to the organization. He brings a strong background in finance, business development, and operational improvement, and customer service to the three centers. Tim received his bachelor's degree from the University of Nevada, Reno and a master's degree from the University of Washington with a concentration on healthcare administration. He is an active member of the Medical Group Management Association, the American College of Heathcare Executives, and the American Society for Reproductive Medicine.
